Our office will file your primary insurance as a courtesy
We do not file secondary insurance. To ensure all information is correctly processed for you, we need a physical insurance card at each visit. A copy of your insurance card and photo id will be made at the time of each visit and also at least once a year, before you see the doctor. Our office will verify your insurance card and general information at each visit. Thank you for your understanding.
